COVID-19 Testing Consent Form

On this form, you are consenting to have your child tested for COVID-19 if they are enrolled in the blended learning model. Please help us keep our PS 178 community safe! Please print, complete and email to your child’s teacher, or hand in to the main office. You can also complete this form digitally by logging into your student’s NYC Student Account.

Walking Trip and Media Consent Form

On this form, you are providing permission for your student to go on walking trips around the community and consent to photograph, film or videotape a student for non-profit use. Please print, complete and email to your child’s teacher, or hand in to the main office.

Emergency Contact Card

On this form (formerly called the blue card), you are providing emergency contact information such as names and phone numbers of contacts and pickup permissions for your child. Please fill out a separate sheet for each child. Please print, complete and email to your child’s teacher or hand in to the main office. Comfort Dog Permission Slip

On this form, you are indicating if your child has permission to interact or be in classroom presentations with our school comfort dog, Taz. Please print, complete and email to your child’s teacher, or hand in to the main office.
